This GivingTuesday, we’re raising funds to give our Enhanced Care Management (ECM) team the support and resources they need to focus on what truly matters: helping clients maintain dignity, independence, and a place in the community they call home.

ECM serves Contra Costa Health Plan members with complex medical and social needs, providing a dedicated provider who integrates clinical care with social support. The program ensures members receive the comprehensive care and guidance they need by providing seven key services:

  1. Outreach & Engagement
  2. Comprehensive Assessment & Care Management Planning
  3. Enhanced Coordination of Care
  4. Health Promotion
  5. Comprehensive Transitional Care
  6. Member & Family Supports
  7. Coordination of / Referral to Community & Social Support Services

Not only is this program beneficial for our clients, but it also saves taxpayers tens of thousands of dollars per person each year!
