This GivingTuesday, we’re raising funds to give our Enhanced Care Management (ECM) team the support and resources they need to focus on what truly matters: helping clients maintain dignity, independence, and a place in the community they call home.
ECM serves Contra Costa Health Plan members with complex medical and social needs, providing a dedicated care manager who integrates clinical care with social support. The program ensures members receive the comprehensive care and guidance they need by providing seven key services:

Comprehensive Assessment & Care Management Planning
Enhanced Coordination of Care
Health Promotion
Comprehensive Transitional Care
Member & Family Supports
Coordination of / Referral to Community & Social Support Services
